Back to Sleep is awarded for killing a total of 300 Fractured Souls during the battle with Zelvraak the Unbreathing in Graven Deep on any difficulty.
Fractured Souls appear in the middle phase of the battle, when the boss reaches 50% health and puts whole group into some limbo state. The souls will be slowly moving from the outside to the middle of the arena. A total of 300 kills can be accumulated over several runs as there won't be nearly enough of souls spawning during a single encounter. However, restarting the battle many times may net you this achievement in just one playthrough.
