It is possible for Nalvyna to refuse you your letter of recommendation in the event that you made certain choices during her quest (such as opening sealed letters, withholding goods, etc). In this case you will need to complete House Hlaalu quests elsewhere in Morrowind before Dreynos Helvi will give you further work.
