Dropped items and newly created items are lost after saving & reloading.
Cause - FormID bugEdit
This behaviour is typical for an unpatched "Shivering Isles" game. The FormID bug was introduced in unpatched versions of SI where dropped and newly created items are lost after saving/reloading because of FormIDs not getting recycled. If you still experience the problem AFTER patching the game, likely either the patching went wrong or something is messed up in your savegame. The patch won't recover previously lost items, but should prevent it from happening thereafter.
Solution: Check your game version on the Title Screen.
Patch "Shivering Isles" from the Official Support website.
Patch the game to the latest version (v1.2.0416) using All the Patches - All Languages.
