In the game files, Carmana's Fight value is set to 80, which means that she will attack you if her disposition is 50 or lower, unless under the effect of a Calm spell - this also applies to the other smugglers residing in Massanud.
A bottle of Telvanni Bug Musk placed outside the cave entrance may give you the Personality boost you need to reach the back of the cave unaccosted.
Valys Anarith in Velonith will mention hearing a splash south of her shack, if asked about a little secret - providing a clue as to Marki's location.
Sarrugi can also reveal Marki's location if asked about Marki the Shark with 60+ disposition.

Marki the Shark is an agent of the East Empire Company, and before he went missing, Marki's work was essential to Company operations. Now Cano wants me to hunt down the Shark.

On his last trip, Marki the Shark was set to meet three clients on the Thirr River: Deldor Arethi in Dondril, Aren Tano in Almas Thirr, and Varuso Veloth in Velonith. Cano wants me to find these merchants and check in on them to get information on Marki's whereabouts.
